APPLICATION

CARDHOLDER/ACCOUNT HOLDER INSTRUCTIONS

A. To purchase from a Merchant Location:

Step 1- Click here or on the picture below to download the Application Form in Word format.  Fill out this Application on your computer and print one (1) copy.  No handwritten Applications will be accepted.

Step 2- Make one (1) copy each of your Cruise Line Crew ID and your I-95 form.

Step 3- Take these copies to your Crew Money Easy Money Transfer Service  merchant along with US $29.95 (Cash Only) plus there is an additional $10.00 when you apply and that goes into your account and is yours. It is used as a reserve which must remain in the account as long as it is active for the purchase of two (2) cards; one (1) Signature-based International Debit Card and one (1) PIN-based International Card . The total of US $39.95 includes a US $10.00 reserve for your Card and belongs to you and remains in your account.

Step 4- This signed copy will be your receipt. Be sure to have the merchant return your copy of the application, Crew ID and I-95 Forms for your records.

Step 5-  Write the Name & Location of the Merchant in the space provided on the Application.

Current International banking laws require that all financial institutions obtain, verify and record information that identifies each person who opens a Bank/Cardholder Card Account with Loyal Bank Limited.  You will be asked to provide your name, date of birth, permanent address, and other information that will allow Loyal Bank Limited to identify you.  You will also be asked to provide documentation as proof of identification. Guaranteed Approval is contingent upon successfully passing this mandatory identification confirmation.

APPLICANT MUST COMPLETE THIS FORM, SIGN IN THE SPACE PROVIDED AND PROVIDE PHOTO COPIES OF YOUR CRUISE LINE ID AND IMMIGRATION & NATURALIZATION FORM I-95 AS PROOF OF ID.

B. To purchase via the Internet:
1. Make a copy of your Crew ID and I-95 forms.
2. Scan them into the computer.
3. Fill out the Application on the “Apply Now� page.
4. Print it out and scan it into the computer.
5. email only the Application as an attachment to:
       a. apps@crewmoney.com
6. Then email the Application, Crew ID and I-95 as an attachment to:
       a. moreinfo@firstesolutions.com
7. Purchase a money order or cashiers check for $29.95 US dollars plus an additional $10.00 and that goes into your account and is yours. It is used as a reserve which must remain in the account as long as it is active, made out to Crew Money LLC and mail it to:

8. Be sure to put the address that you want the two cards (International Debit Card And International ATM Card) mailed to on the Application.
9. You will receive your cards within 10 to 14 days.
